ACA 1095-C Report

The 1095-C report is crucial for generating your employees' 1095-C forms and preparing the necessary file for submission to the IRS. Employees receive a 1095-C form if their employer qualifies as an Applicable Large Employer, subject to the employer shared responsibility provisions under the Affordable Care Act. This Form 1095-C contains information regarding the health insurance coverage offered to employees by their employer.


To generate a 1095-C report, navigate to the "Reports" module and click "Company Reports"

Before generating the report:

  1. Establish your report settings:
    1. Select the Setting [Gear] button, complete with all necessary information, and click save when complete.

Once the settings have been established, you can now work towards generating your report.

  1. Click on the green [Play] button


  2. Select your parameters
    1. Year
    2. Employee: (Leave blank for all employees)
    3. Show Form 1094-C (Toggle on/off)
    4. Show form 1095-C (Toggle on/off)
  3. Once parameters are selected, click on 'Build Report'
  4. Once your report is built, you can either 'Download' or 'Create File'
  5. Create File:
    1. A zip archive that contains two files, Manifest File and Form Data File, will be generated. These are the files that have to be uploaded to the IRS portal.
